These forms are provided to clients for their information and for review prior to a mediation meeting. Clients review and sign the Statement of Confidentiality form and the Agreement to Participate in Mediation form at the start of the mediation meeting to indicate their understanding and agreement to participate in the process. They use the Mediation Agreement form to record their resolutions.
